Diabetes on the Move! is a 10-hour
self-management program accredited by the State of Iowa and
recognized by the American Diabetes Association to assist persons
with diabetes mellitus maintain an active lifestyle and prevent
damage from the complications of diabetes. Classes are held on
three half-days or evenings, times vary according to the needs of the clients.
Topics covered include:
- Managing blood glucose
- Recognizing signs, and treatments of high and
low blood sugars
- Establishing a home exercise program
- Learning healthy eating habits
- Learning to read food labels
- Learning the complications of diabetes and
ways to prevent them
- Selecting and using a glucometer
- Managing blood pressure
- Learning skin, foot and dental care
- Learning to take medications safely
- Identifying support services
- Learning what to eat in restaurants
Diabetes on the Move! support groups are
held four to six times per year. Call for the next group.
The support group is free to all that have diabetes. The
meeting consists of a 30-45 minute presentation by a professional,
followed by general "networking". Topics previously covered
include: eye care, foot care, and healthy cooking.
Individual
Services
Diabetes Self-Management Training Program A series of 3-three and a half hour programs designed to teach the
participant how to manage his own diabetes. Days and time are
arranged to meet the needs of the clients.
Medical Nutrition Therapy One-on-one consultation with a Registered Dietitian and/or Certified
Diabetic Educator to design a healthy eating pattern to fit your
lifestyle and meet your individual needs.
Insulin Instruction An individual session for persons who have already completed
Diabetes Self-Management Training, but now need insulin injections
to control their diabetes.
Diabetic Screenings Diabetic screenings are conducted periodically for the public and at
businesses. If you would like to schedule a screening for a

Research shows that persons who meet the
American Diabetes Association's goal of Hemoglobin A1c of
less than 7.0% (an average blood sugar of 140) have 76%
fewer eye problems, 56% less kidney damage, 60% less nerve
damage and fewer amputations. Persons who received
self-management training are much more likely to achieve
these goals.

Medicare covers two hours of Medical Nutrition
Therapy per year and 10 hours of Initial Diabetes Self-Management
Training to each participant. Medicare also covers an
additional 2 hours of training each year.
Other insurance programs vary in their coverage.
Some require pre-certification so it is best to contact your
insurance carrier through the 800 phone number on the back of your
card.
