We believe that many people benefit, both emotionally and physically, from remaining in their own home while recovering from an injury or illness.

SMC Home Health offers a wide range of therapies and nursing care that can be brought to you in your home.

As a follow-up to your hospital visit, Home Health can provide the medical support necessary to complete your hospital plan of care and help assure your continued improvement.

When an injury or illness occurs that does not require hospitalization but does need medical support, your physician, with the assistance of your home health team, can design a care plan to meet your specific needs.  Our registered nurses then provide nursing care and coordinate other aspects of your care with members of your team.

 

Services and Therapies

Dressing Changes and Wound Care
Blood draws, IV therapy, medications, tube feedings, and other treatments are provided by our trained staff.

Infusion Therapy
With our portable equipment, it can be brought to your home.  Infusion therapy provides an array of options in the home.  Each infusion therapy is managed by a registered nurse under the guidance of your physician.

Nutrition Therapy
Such as Total Parental Nutrition, Enteral Nutrition, and Hydration Therapy, provides nutrition to the person who has difficulty swallowing or needs nutrients.

Antibiotic Therapy
Provides antibiotics to be intravenously infused to help medically stable patients fight off a potentially serious infection.

Chemotherapy
Cancer Treatment can be emotionally and physically exhausting.  Being in the familiar surroundings of your home, with your family around your for support, can be very important at a time like this.  On given occasions, your physician, with approval from your insurance carrier or Medicare, Medicaid or VA, may order chemotherapy provided in your home.  In these instances, Home Health can provide the technical and nursing services necessary for this treatment.

Pain Management
Provides pain relievers administered continuously or in a cycle to ensure the most comfort possible for patients with unyielding pain.  Your nurse will work with your and your physician and family to provide the most comprehensive support possible.

Physical Therapists
Will visit your home and provide instruction on exercises and massage techniques to improve mobility, strength, and restore functioning.

Occupational Therapists
Will help you develop strength and dexterity for the daily skills needed to restore as much independence as possible.

Speech Pathologist
Will guide you in relearning speaking and other communication skills.  Dysphagia (Swallowing) Therapy is also offered.

Client and Family Education
In all areas of health care, including diet, diabetes, ostomy care, and oxygen therapy, is provided by our healthcare professionals.

Parenting and Newborn Needs
Follow-up visits after delivery can be provided by our nurses in your home.  Instructions in the care of your newborn, special treatments, and simple reassurance can help provide the peace of mind so important at this time.

 

Home Health Team

Registered Nurses
Our nurses coordinate your care with your physician and other team members.  They provide regular nursing visits for care and instruction, make assessments of your health status, and report this to your physician.  Clients maintain their relationship with their family physician. 

Home Health Aides
Our aides are important to assisting in your independence.  They provide:

  • Personal Care Services such as bathing, dressing, and nail care.
  • Light housekeeping, laundry, and meal preparation.
  • Respite for family members who are caregivers.
  • Client and Family Instruction.

 

Financial Coverage

Most major insurance companies, as well as Medicare, Medicaid, and VA, cover the cost of these services for qualified participants.
