Hospice is a healthcare service that provides compassionate
support to individuals and their families who are coping with an end-stage illness.  Our professional staff and volunteers assist patients in making the most of life and in retaining control of the decisions affecting them.  Patients and families select the Hospice support services they need and oversee all aspects of the care plan, including their activities of day-to-day living.
Our goal is to help individuals live in dignity while the end of their life comes naturally and peacefully.


Services at SMC Hospice

SMC Hospice serves individuals who:

  • Have been diagnosed with any end-stage illness, including cancer, heart, lung, and liver disease, AIDS and Alzheimer disease
  • Seek symptom control and comfort care
  • Are certified by their physician to have an end-stage illness

Pain and symptom control lie at the very heart of our Hospice program care.  Medical and drug therapies make possible a control of pain that enhances the quality of life in the Hospice setting.  Medical expertise and an individualized care plan manage a range of debilitating symptoms which, when left untreated, can diminish a patient's strength and human dignity.
 

You Can Expect

  • Compassionate, comfort care.
  • Pain and symptom control.
  • A nurse and social worker available 24 hours a day, 7 days a week.
  • Instruction on how to care for the patient's needs.
  • Counseling services to enhance your coping ability and ease emotional distress for the patient and family.
  • Home health assistance, light housekeeping and personal hygiene care of the patient.
  • Dietary assistance, special therapies and pharmaceutical counseling.
  • Spiritual guidance upon request, through hospice chaplains who work within patients' own faith communities or who are available for those who have no religious affiliation.
  • Companionship and support from hospice volunteers, as requested.
  • Educational and bereavement counseling to continue for family members after the death of a loved one.
  • Members of the hospice team will inform and involve you and your family and respect your wishes in the care of a loved one.
  • Information that can help you make informed decisions about complementary care options.

Financial Coverage

Care through SMC Hospice is almost always covered by Medicare, Medicaid, private insurance or other third party payers.  This includes medications, medical equipment and supplies to make the patient more comfortable.
 

Team Members

With SMC Hospice, a team of highly trained professionals and volunteers are nearby to listen and support your needs.  Each member is uniquely experienced in dealing with end-of-life issues.

Team members include Physician as Director of Care, Nurse, Chaplain, Dietitian, Social Worker, Pharmacist, Home Care Aide and Volunteers.
 

Donate to SMC Hospice

Gifts given to SMC Hospice may be designated to assist in special projects which will develop and enrich the care given to future Hospice patients.

Contributions will address the growing need for services like comfort care, grief counseling, family bereavement and caregiver assistance as well as special equipment and on-call nursing care in the coming year.  The ultimate goal is to establish a library that will provide educational resources for the Hospice patient and the patient's family.
